The Faculty Handbook, Chapter 5, provides information and guidelines related to the dean or peer review of teaching materials.  This form is the current teaching materials review document.

Faculty Development Fund Application

FDF Application 2026-2027 (Word Document)
The Faculty Development Funds Review Guidelines are included in Appendix 3 of the Faculty Handbook.  This form is for 2026-2027 FDF applications.

FSI Form (Word Document)
The Faculty Handbook, Chapter 5, provides information and guidelines related to the FSI.  This form is the current FSI document. (Revised May 2026)

Peer Evaluation of University Service

Peer Evaluation of University Service (Word Document)
The Faculty Handbook, Chapter 5, provides information and guidelines related to university service.  This form may be used to evaluate a faculty member’s service on a university committee or task force. 

Sabbatical Application Documents

Sabbatical Application 2027-2028 (Word Document)
The sabbatical application policies and procedures are included in the Faculty Handbook, Chapter 4.  This form is for 2027-2028 sabbatical award applications that are due October 1, 2026.  (Revised May 2026)

Sabbatical Application-Dean’s Statement of Support (Word Document) This form is a required attachment to the Sabbatical Application and must be completed by the applicant’s dean.

Sabbatical Leave Award Reporting Form (Word Document)
Faculty receiving a sabbatical leave award are required to complete this form to document work completed for institutional assessment and program effectiveness and to retain eligibility for future awards. Please submit this form within the first four weeks of the semester following the sabbatical leave to the provost at provost@uiwtx.edu.

Syllabus Template

UIW Syllabus Template (Word Document)
This template has been designed to ensure accessibility compliance and has been approved by the Faculty Senate as our official course syllabus template. Please use it when creating a new syllabus or proposing revisions. (Revised January 2026)

Notes: 

  • Highlighted text constitutes either instructions, which should be deleted when the syllabus is completed, or options, of which one or more can be deleted as appropriate. 
  • Additional tables containing information relevant to accreditation or related standards may be included after the “Course Outcomes and Assessments” table.
  • The header should be noted “Approved by UC” for undergraduate syllabi or “Approved by GC” for graduate syllabi.
