This page houses many of our internal process forms for the Office of Student Advocacy and Accountability (OSAA). Each of the links below will direct you to the form and each form will have directions about the use, intended purpose, and/or limitations associated with its use. Some of our forms require you to log-in with your UIW credentials to ensure that you, the student, is filling out the form.
All forms should be filled out with complete, accurate, and specific information. Please know that completing this report with incomplete or inaccurate information may impair, deter, or delay our ability to appropriately process your form/request.

Forms
Behavioral Misconduct
Acknowledgement of Rights and Expectations - As University of the Incarnate Word students, you are entitled to certain rights when you speak with University officials regarding an investigation into possible violation(s) of University policy. Students should complete this form prior to meeting with their Hearing Officer.
Appeal of Case - This form should be used to file an appeal in your case with OSAA or Residence Life staff. Appeals can only be filed once, and under specific grounds. Please make sure you have your decision letter with the name of your Hearing Officer when filling out this form.
Sanction Submission Form - This form should be used to submit sanctions for review by your Hearing Officer. Please make sure you have your decision letter with the name of your Hearing Officer when filling out this form.
Access to Records
FERPA Release Form - This release grants permission for designated individuals to participate in the conduct process and/or review of the contents of your conduct records. Please note that access to your records does not grant permission to alter the record or conduct business on your behalf.
Verification of Conduct Records - This form is to request the verification of conduct records by a UIW student. This form should be completed by the student needing the verification. Any documents that need to be completed by UIW staff should be included in your submission.
