Forms from the Registrar's Office

You may download these forms for conduct business with the Registrar's Office. Because of privacy legislation, we cannot accept forms from students by e-mail attachment. You will need to download and complete the form and then mail or deliver it to the Registrar's Office. If signatures are required on the form, you must obtain those signatures before sending the form to the Registrar's Office. Some forms require student identification and cannot be sent to us through the mail but must be delivered to the Registrar's Office so that we can confirm the identification.
