Forms for Faculty
You may download these forms to conduct business with the Registrar's Office. After completed, send the form to the appropriate approval entity (e.g., your dean, school/college curriculum committee, etc.) prior to routing to the Registrar's Office.
For course syllabi, course outlines, and/or course/program curriculum processing forms, contact the chair of the Undergraduate Curriculum Committee, the Graduate Council chair, or the Faculty Senate President for the current form. Forms are also available in Blackboard.
