You may download these forms to conduct business with the Registrar's Office. After completed, send the form to the appropriate approval entity (e.g., your dean, school/college curriculum committee, etc.) prior to routing to the Registrar's Office.
For course syllabi, course outlines, and/or course/program curriculum processing forms, contact the chair of the Undergraduate Curriculum Committee, the Graduate Council chair, or the Faculty Senate President for the current form.
